      To proceed, print this page and use this checklist for gathering documents and supporting evidence, and complete the questionnaire. Then, send by way of mail or courier service, all parts of this questionnaire with all requested supporting documents to:

      The following supporting documents are required to begin the process; use this checklist to check off each item as it is gathered:

      _______Copy of all I-797's (Approval Notices) if you have ever entered the U.S.
      _______Copy of all valid CGFNS certificates and/or NCLEX certificates and/or State licenses.
      _______Copy of all Diplomas or Certificates and/or Degrees you may hold.
      _______Copy of your academic transcripts or course descriptions for degrees or diplomas.
      _______To verify your past professional experience, provide copies of employment verification letters from prior employers, if available.
      _______Copy of your marriage/or divorce certificate, if applicable
      _______Copy of birth certificates for you, your spouse and children under 21 (if applicable).
      _______(If you have ever applied for a green card before) copies of all I-140, I-485 receipt notices or approval notices, or Packet 3 notice
      _______(If available) copy of your Visa Screen certificate (not required for Phase I but is helpful to include in the petition if available)

      We must have information requested in all parts of this form in order to successfully complete the I-140 Petition. We may request further information or clarification of information at a later date if necessary. If a problem arises that may affect your immigration petition, you must let us know immediately.

      8. Very Important. On a separate sheet of paper, give an accounting of your employment for at least the past three (3) years. Include all employers starting with your current employer (not different assignments you had while working for your employer). Also include time you took off between jobs and explain that time (vacation, seeking employment, etc.). If you were in school, indicate that also. The INS is very particular about your work experience for the previous three years. Give all the following information:
